I, (Participant), acknowledge that I understand and agree to abide by the policies and procedures set forth by the Senate Football Fieldhouse (Facility). I understand that participation in sports activities involves inherent risks of serious bodily injury, including permanent disability, paralysis, and death. I am voluntarily participating in these activities with knowledge of the dangers involved and hereby agree to accept any and all risks of injury or death.
I acknowledge and understand that the Facility assumes no responsibility for any injuries or damages sustained while participating in sports activities on its premises. I agree to release, indemnify, and hold harmless the Facility, its owners, employees, agents, and representatives from any and all claims, demands, causes of action, damages, or liabilities of any kind arising out of my participation in sports activities on the Facility's premises.
I understand that the Facility has provided me with a safe and properly equipped facility for my use. However, I acknowledge that there are certain inherent risks involved in sports activities that cannot be eliminated, reduced, or controlled, and that accidents and injuries may occur despite the best efforts of the Facility, its owners, employees, agents, and representatives.
I also understand and agree to comply with all rules, regulations, and guidelines set forth by the Facility and any applicable sports organizations governing my sport. I understand that failure to follow these rules may result in revocation of my rental privileges or other disciplinary action.
I agree to assume full responsibility for any and all risks, injuries, or damages known or unknown, which I may incur as a result of participating in sports activities on the Facility's premises, including but not limited to, broken bones, muscle strains, muscle pulls, muscle tears, heat prostration, injuries to knees, injuries to back, injuries to foot, or any other illness or soreness, including death.
I knowingly and voluntarily waive any claim I may have against the Facility or its owners, employees, agents, and representatives for injury or damages that I may sustain as a result of participating in sports activities on the Facility's premises.
I have read and fully understand the terms of this Waiver of Liability and Assumption of Risk, and I voluntarily agree to its terms. I also affirm that I am at least 18 years of age or that I have obtained the permission of my parent or legal guardian to participate in sports activities on the Facility's premises.
I acknowledge that by signing this document, I am giving up certain legal rights and remedies which may otherwise be available to me.
